Masters Provincial Championship Teamsheets

The Masters Provincial Championships has online teamsheets specially designed for the competition. This teamsheet is a fillable PDF form. A team official must fill out the teamsheet and then print one copy which must be signed by all players and team officials.

For Masters Provincial Championship games:

  • A representative of each team shall, no later than 20 minutes prior to the kick-off time, hand to the referee, his/her team's teamsheet along with the "Teamsheet - Referee Report".
  • The teamsheet shall list all eligible players (including potential substitutes) and up to five team officials.
  • The teamsheet must be signed by all players and team officials.
  • After checking the teamsheets for proper completion, a representative of each team may take a photograph of the teamsheets prior to the game. Paper copies of the teamsheets will not be provided to the teams at the field.

Referee's Report

The Referee Report is the second page of the signed teamsheet (mentioned above). A referee report must accompany each team's teamsheet.  These reports must be completed and sent along with the teamsheets to Ontario Soccer.
