Submit Your Applications For Meritorious Service Awards

This year's 2015 Meritorious Service Awards will be presented in Toronto on April 25, 2015. The submission deadline is Tuesday, March 31, 2015.

The Centre Circle Awards are The Ontario Soccer Association’s highest level of recognition.

The Centre Circle Award winners exemplify those who have made outstanding contributions to the game.

AWARDS CATEGORIES

Life Membership
An individual who has been associated with The Association or with one of its Members, and who has rendered special service to the game of soccer in Ontario to an extent beyond normal participation. The nominee must also have served on The Association’s Board of Directors.

Corporate Award
A Corporation, Municipality or Company which has rendered significant contribution in finances goods or services to The Association or its Members for a period of not less than three years.

Media Award
An individual who, or organization which has provided significant news media coverage of the programs of the OSA, its Members or affiliated leagues for a period of not less than less three years.

Meritorious Service Award (Club) 
A Soccer Club which by its existence and operations has made significant contribution to the development of the game of soccer and has enhanced the quality of life in its community for a period of ten years or more. The club must be in good standing with the District Association in whose area the Club has designated its headquarters. The Club nomination must be approved by the Club’s District Association.

The awards nomination must include references from:

  1. The District Association
  2. The league(s) in which its teams play, and
  3. One other from within its community
  4. Details of exceptional achievement and a Club constitution must accompany the nomination.

Meritorious Service Award (Individual) 
An individual who, over an extended period of not less than twenty years, has rendered special service to the game of soccer and who has dedicated her/himself during that time to the assistance of others and the promotion and development of the game in one or more Districts in the province. If an active player, the individual must not be under suspension when nominated. Please note that this category does include coaches and referees.
