Mandatory Waiver

NEW for 2009

Starting in 2009, all referees or Parents/Guardians of the referee will have to sign either a Participant's Agreement (if referee is u18 by the date of registration) or a Waiver (if 18+ by the date of registration).

These forms have been mandatory for players and coaches to sign for the last three years, and the referees must complete one again this year. The first draft of the waiver was much stricter, however, has it been vented and redrafted before publishing.

When you go to register in 2009 please make sure you sign the appropriate form or you will not be able to be registered with the OSA.
